Answer Table 1 first, then Table 2 and then compare the scores.

Remember, there is no right or wrong answer, or traits that are “bad” or “good”. This is also not a “wish list” of how you would like to be. Be
objective in your scoring. When you feel that a certain characteristic applies to you 50% of the time, then mark it.

(Example:  if you are 50% late for appointments, then mark it)

If you agree with a statement, tick the box and score yourself 1 point. Score yourself 0 if the box is not ticked. Add up your score and fill in the blank space at the bottom.
